- You Are 16 or over
one of two: - You Are citizens of New Zealand
- You Are permanent residents, or
- have applied for or have applied for refugee or protection status

Community Services Card Discounts
Community service cards can reduce the cost of:
- Prescription fees
- Fee for visits to health practitioner after hours
- Eyeglasses for children under 16
- Visit a health practitioner (eg, a doctor or nurse) if you are enrolled in their practice (for yourself, and for your dependent children aged 14-17)
- ACC goes to a general practice if you get injured (you may not need to be enrolled in their practice – it depends on the practice)
- Emergency dental care is provided by:
- Hospital, and
- Approved dental contractor (ask the dentist if they are an approved contractor)
- Travel and accommodation You have been referred outside your area for treatment at a public hospital:
- At least 80km for adults
- At least 25 km for children

- Download the Community Service Card Application Form
- Call on 0800 999 999 and ask for a
- Call the Work and Income Service Center
- Ask a health practitioner, for example, your family doctor or pharmacy.
