How to Applying for Valenzuela.gov.ph Health Card?
1. To begin Your Health Certificate online application, please visit www.valenzuela.gov.ph and click on the 3S+ Online Services button.
2. Fill up your Profile information Double-check the details before proceeding.
3. You have finished the first step of the application and check your email to proceed with the next part.
4. Click the URL Link found in the email to proceed to the exam.
5. Please take note that you can only take the exam THRICE and once you fail, you will only be allowed to retake the exam after 24 hours.
6. Watch the video and understand the topics covered.
7. Take the exam.
8. If you fail the exam on your first try, click try again and if you fail the exam thrice, wait 24 hours before trying to retake the exam.
9. If you passed the exam, you can now proceed to payment.
10. On the Health Card Payment, click Terms and Conditions on your selected payment gateway.
11. Select the preferred language on the Terms and Conditions and Tick the box indicating your agreement to the Terms and Conditions.
12. You may now proceed to your payment.
13. Once payment is completed, You may proceed to your profile.
14. Go back to your Profile page to see the Laboratory Appointment schedule.
15. You can only apply for a new health card once it has expired or if you are transferring to a new company.
