- First, you visit this website sects.up.gov.in/public/starttoapply.aspx
- After filling in your mobile number which is linked with your Adhar card, fill up the captcha, and click on the “Generate OTP” button.
- On entering/verifying the OTP, the UP State Health Card Online Application Form will appear:-
- Enter all asked details correctly and then click on the “Submit” button to complete the UP State Health Card Online Process.
UP State Health Card Requirement Eligibility
- People should be permanent residents of Uttar Pradesh.
- Only the employees of the Uttar Pradesh government can get the benefits of the scheme.
- The benefit of this scheme can also be availed by the pensioner’s employee of the state.
- Pensioners can also join the scheme.
- To join the cashless medical scheme, each beneficiary family will have to pay a fee of 1102.
